ABSTRACT
Purpose:To report a prospective feasibility study of Cutting Balloon angioplasty (CBA) applied in the predilation phase carotid artery stenting (CAS) highly calcified lesions. Methods:From January 2003 to February 2007, 178 consecutive patients (109 men; mean age 73.1±7.3 years) with lesions underwent CAS CBA as pre-specified strategy procedure. All steps procedure were performed under cerebral filter protection. The cutting balloon ranged diameter from 3 4 mm and was inflated at nominal pressures target lesion. Pre-CBA dilation low-profile coronary only when not able cross Selection filters stents operator's discretion. Primary endpoints all stroke death rates 30 days 6 months. Secondary included success (positioning full inflation), technical (residual angiographic stenosis <30%), procedural (technical no complications), in-hospital major complications. Results:Cutting achieved patients. In 32 (18.0%), pre-CBA necessary due inability lesion initially. One (0.6%) patient suffered transient neurological intolerance flow cessation massive debris distal filter; this event completely resolved after removed (CAS 99.4%). day 15 (0.6% 30-day rate). At 6-month follow-up, 174 (97.7%) evaluated; 1 died myocardial infarction 35, 2 non-neurological or cardiac causes 103 158. cumulative rate 2.2%. Conclusion:These data suggest that during is safe useful method prepare subset for stenting.
