A New Neuroanesthetic Protocol of Rendering Specialized Care in Treating Degenerative Lumbar Spine Diseases in High-Risk Patients: Prospective Analysis of the Results

ABSTRACT
The aim of the study is to assess effectiveness a new neuroanesthetic protocol for treating degenerative lumbar spine diseases in high-risk patients.Materials and Methods.Two groups patients with high risk anesthesia surgery determined by authors' clinical decision support system (CDSS) have been prospectively studied.A was used experimental group (EG, n=25), while control (CG, n=25) underwent intravenous based on propofol fentanyl.Minimally invasive transforaminal interbody fusion performed all cases.Changes intraoperative mean arterial pressure heart rate, intensity local pain syndrome, amount opiates used, presence cognitive disorders, adverse effects anesthesia, surgical complications compared.Results.The were representative (p>0.05) terms age-gender parameters, anthropological data, comorbid background, involvement smoking, preoperative characteristics spine, as well level functions.No statistically significant changes (p=0.17) registered EG relative CG (p=0.0008).Intraoperative reduction rate not noted (p=0.49) comparison (р=0.03).In postoperative period, best indicators functions FAB test (p=0.02)and MoCA (p=0.03)were revealed EG.A significantly less perioperative (p=0.005) at low syndrome also (p=0.01).The intergroup analysis has shown fewer compared (p=0.01) comparable number (p=0.42). Conclusion.A rendering specialized care surgery, assessed authors-developed CDSS, resulted effective elimination application opioids, stabilization cardiovascular activity.In addition, no anesthetic side-effects, pharmacological consequences complex usage non-steroidal anti-inflammatory drugs, prolonged anesthetics, alpha-2-agonist, non-narcotic analgesics registered.
