Advancing Breast Cancer Care in Octogenarians: A Personalized and Multidisciplinary Management to Better Outcomes

ABSTRACT
Background: Breast cancer in women aged 80 years and over accounts for about 12% of cases, but its management remains difficult due to the heterogeneity this population absence relevant evidence-based guidelines. Treatment decisions must integrate considerations biological age, comorbidities, life expectancy, therapy-related toxicities alongside tumor biology stage. This study aims evaluate clinical outcomes elderly breast patients treated at our Institution through a multidisciplinary approach that includes oncologists, surgeons geriatric specialists. Materials Methods: retrospective analysis reviewed records ≥80 Fondazione Policlinico Universitario Agostino Gemelli IRCCS Rome, Italy, from January 2016 December 2020. The examined clinicopathological data, surgery, adjuvant therapies outcomes. were guided by evaluations involving surgeons, Onco-geriatric assessments (GA) performed all treatment feasibility associated risks. Primary included overall survival (OS), disease-free (DFS), cancer-specific (CSS). Surgical treatment-related complications also documented. Results: A total 238 study. Of these, 203 (85.3%) underwent breast-conserving while 35 (14.7%) mastectomy. Axillary surgery was 129 (54%) cases. Regarding treatments, 93 (39.1%) received radiotherapy, 101 (42.4%) endocrine therapy alone. Chemotherapy administered six high-risk following GA, with no reported toxicities. Over median follow-up 42.3 months, one local recurrence (0.5%), regional node 19 cases distant metastases (9%). (9%) died cancer. complication rate low, 10% experiencing wound dehiscence, hematoma, lymphedema, or similar issues. Five-year as follows: OS 73.3%, DFS 66.6%, CSS 88.5%. Conclusions: highlights yields favorable outcomes, low recurrence, metastasis, rates. personalized strategies, onco-geriatric assessments, balance benefits quality minimizing risks overtreatment undertreatment.
