ABSTRACT
Medical questionnaires and forms play a crucial role in diagnosing diseases gathering essential patient information. Traditionally, these assessments are conducted through face-to-face interactions or phone calls, which can be time-consuming, costly, inefficient. To address challenges, digital adaptations of medical have been developed, enabling faster more accessible data collection across various platforms applications.ResearchKit, an open-source framework introduced by Apple, allows researchers developers to create robust mobile applications for research. By leveraging this technology, large-scale efficiently, facilitating real-time analysis while reaching broader population. The availability extensive datasets is computational techniques such as Machine Learning, require significant amounts classification, pattern recognition, predictive modeling healthcare.This paper focuses on the digitization questionnaires, specifically International Physical Activity Questionnaire (IPAQ) Mini-Mental State Examination (MMSE), using ResearchKit. transition from paper-based significantly improves efficiency assessments, allowing healthcare professionals analyze results remotely optimize diagnosis treatment plans. findings emphasize impact health solutions advancing research, reducing costs, enhancing accuracy scalability data-driven applications.
