Renal malignant solitary fibrous tumor with single lymph node involvement: report of unusual metastasis and review of the literature

ABSTRACT
Solitary fibrous tumors are rare mesenchymal spindle cell neoplasms that usually found in the pleura. The kidneys an uncommon site and only few cases of renal solitary tumor exhibit malignant behavior metastasizing to liver, lung, bone through hematogenous pathway.To describe first case lymph node metastasis from order increase knowledge about these tumors.A 19-year-old female patient had intermittent hematuria for several months without flank pain or other symptoms. A chest abdomen CT scan was performed showed a multi-lobed bulky solid mass 170 × 98 120 mm left kidney. One day before surgery, artery catheterized kidney embolization using Haemostatic Absorbable Gelatin Sponge polyvinyl alcohol. We then radical nephrectomy with hilar, para-aortic, inter-aortocaval lymphadenectomy.Estimated intraoperative blood loss 200 mL operative time 100 minutes. No postoperative complications occurred. hospital stay 7 days long. histological examination Cancerous tissue cellular atypia, increased mitotic index (up 10 hpf). Immunohistochemical analysis positive results CD34, BCL2, partial expression HBME1, occasionally synaptophysin. Histological evaluation confirmed presence one hilar node. did not receive any therapy. At 30-month follow-up, good health no local recurrence metastases occurred.This is showing unusual behavior; this finding adds new information biology progression tumors, which remain unclear.
