ABSTRACT
Background and purpose: Avascular necrosis (AVN) is one of the common complications after closed reduction hip spica cast for developmental dysplasia (DDH). Prereduction traction has been used to reduce a dislocated or decrease risk AVN, but there are conflicting results in prevention effects on AVN. The purpose this study was systematically review current literature evaluate effect prereduction preventing AVN children with DDH treated by through meta-analysis. Materials methods: A systematic performed using PubMed EMBASE variations three major terms: 1) dislocation; 2) reduction; 3) avascular necrosis. Seven studies that could compare incidence between no-traction group were included. Methodological quality assessed, heterogeneity test done ( p =0.008), pooled ratios estimated. Results: association data 683 hips reduction. groups ranged from 5% 47.7% 0% 72.7%, respectively. meta-analysis random model indicated no significant difference =0.536). Conclusion: There insufficient evidence decide efficacy before reducing patients To recommend long-term follow-up considering age, severity dislocation, appropriate method needed.
