ABSTRACT
Liver transplantation (LT) is indicated in patients with severe acute or chronic liver failure for which no other therapy available. With the increasing number of LTs recent years, centers worldwide must manage their according to clinical situation and expected waiting time transplantation. The LT clinic at Centre hospitalier de l'Université Montréal (CHUM) developing a new health care model across entire continuum pre-, peri-, posttransplant that features patient monitoring by an interdisciplinary team, including accompanying patient; digital platform host plan; learning program; data collection from connected objects.
