Views and Uses of Sepsis Digital Alerts in National Health Service Trusts in England: Qualitative Study With Health Care Professionals

ABSTRACT
Background Sepsis is a common cause of serious illness and death. management remains challenging suboptimal. To support rapid sepsis diagnosis treatment, screening tools have been embedded into hospital digital systems to appear as alerts. The implementation alerts improve the deterioration complex intervention that has fit with team workflow views practices staff. Despite importance human decision-making behavior in optimal implementation, there are limited qualitative studies explore experiences health care professionals regarding or computerized clinician decision (CCDSSs). Objective This study aims on use CCDSSs identify barriers facilitators their National Health Service (NHS) hospitals. Methods We conducted qualitative, multisite unstructured observations semistructured interviews from emergency departments, outreach teams, intensive acute units 3 NHS trusts England. Data both were analyzed together inductively using thematic analysis. Results A total 22 interviewed, 12 observation sessions undertaken. four themes identified: (1) nested electronic records, but never substitute professionals’ knowledge experience; (2) remind take action according context, such unit job role; (3) introduction, by making them more accessible, easy use, not intrusive, accurate, well integrated across whole system; (4) contextual factors affecting trusts. Digital optimally used general lower senior maker:patient ratio experience similar technology. Better was associated quality improvement initiatives continuous training. trusts’ features, presence 24/7 team, good technological resources, staffing teamwork, favored use. Conclusions Trust requires multiple levels at all phases intervention, starting prego-live analysis addressing organizational needs readiness. Advancements toward minimally disruptive smart CCDSSs, which accurate specific same time scalable require policy changes investments multidisciplinary research.
