ABSTRACT
Background: Congestive heart failure (CHF) is a major public health issue. Today, CHF affects 6.5 million people in the U.S. and incidence rate projected to rise by 46% more than 8 cases 2030. Current reimbursement policies use readmission rates length of hospital stays as indicators quality care, incentivize providers meet these measures cost hospitalization alone significantly contributes overall burden on patients systems. Symptoms can be unpredictable presently there are no reliable solutions track disease control for discharged patients.
