ABSTRACT
Purpose: In 2012, a revised guideline for acute acetaminophen overdose was proposed in the UK, recommending that treatment threshold should be lowered to 100 mcg/ml at 4 hours after ingestion without risk stratification of hepatotoxicity. However, poison centers some developing countries do not have laboratory resources provide serum drug levels time. The primary aim study is evaluate cut-off value reported dose per kilogram determine when N-acetylcysteine warranted under guideline. Methods: Data were collected retrospectively from toxicology registry an urban emergency medical center between 1st January 2010 and 30th June 2017. Inclusion criteria single more than 75 mg/kg 15 over 14 years age. Subgroups created by 25 increments dose, then sensitivity, specificity, positive predictive negative calculated values mg/kg, 125 150 175 toxic level '100-treatment line'. Results: A total 99 patients enrolled study; 24 showed (24.2%). Zero 17 with (0%), 0 2 (14.3%), 12 (33.3%) had levels. higher ingested weight, frequency concentration on first test (${chi}^2$ trend, ${chi}^2=22.66$, p-value<0.001) sensitivity each 100%, 92% 76%. Conclusion: intoxication, weight will useful determining need antidote indigent environment.
