ABSTRACT
We report a case of unruptured “true posterior communicating artery” (Pcom) aneurysm associated with ruptured internal carotid-posterior artery (IC-PC) in 72-year-old woman, treated successfully by direct surgery. Left carotid angiography and three-dimensional CT revealed an IC-PC 8 mm maximum diameter protruding inferolaterally, small bulge on the Pcom side which was regarded as part aneurysm. Intraoperative inspection that prominence initially thought to be true forming kissing pair The 2 aneurysms were completely dissected both clipped. This is first reported A aneurysm, present case, may difficult diagnose preoperatively. Careful intraoperative observation complete dissection are therefore important for good outcome.
