ABSTRACT
OBJECTIVE: To evaluate commercially available determination methods for HbA1c in patients with hemoglobin variants. RESEARCH DESIGN AND METHODS: values were determined various methods, including ion-exchange high-performance liquid chromatography (HPLC), boronate affinity assay, and immunoagglutination the mutations Hb Graz, Sherwood Forest, O Padova, D, S. RESULTS: The effect of hemoglobinopathies on glycohemoglobin measurements was highly method dependent. HPLC lacked resolution necessary to differentiate They demonstrated additional peaks chromatograms results either too low or high compared nondiabetic reference range. With all immunoassays, Graz falsely values. other our study caused and/or methods. showed an acceptable range CONCLUSIONS: Because local occurrence variants ethnic origin a given population, every individual laboratory must establish validate its own assay method. In managing diabetic patients, knowledge influencing is essential because could cause mismanagement diabetes resulting from false determinations.
