Factors that Differentiate between Endometriosis-associated Ovarian Cancer and Benign Ovarian Endometriosis with Mural Nodules

ABSTRACT
Mural nodules and papillary projections can be seen in benign ovarian endometriosis (OE) malignant transformation of OE (endometriosis-associated cancer [EAOC]), which pose a challenging diagnostic dilemma to clinicians. We identify the preoperative imaging characteristics helpful differential diagnosis between with mural EAOC.This was retrospective study 82 patients who were diagnosed pathologically have (n = 42) transformations these tumors 40) at Nara Medical University Hospital from January 2008 2015. All assessed contrast-enhanced MRI before surgery. Patient demographics, clinical pathologic features analyzed detect significant differences two groups.Histological examinations resected tissue specimens revealed that majority (78.6%) nodular lesions retracted blood clots. found nodules, when compared those older, had larger cyst diameters nodule sizes, more likely exhibit taller than wider lesion. They also present various signal intensities on T1-weighted images (T1WI), high-signal intensity T2-weighted (T2WI), lower proportion shading T2WI, show an anterior location cyst. In multivariate logistic regression analysis, "Height" (>1.5 cm) "Height-Width ratio (HWR)" (>0.9) maximum diameter (>7.9 cm), age (>43 years) independent predictors distinguish EAOC nodules.The "HWR" may yield novel potential factor for differentiating nodules.
