ABSTRACT
Bilateral trigeminal neuralgia refractory to medical therapy is a rare occurrence and it mandatory choose therapeutic procedures minimizing possible bilateral sensitive deficit due the employment of mininvasive ablative techniques. A patient affected by secondary multiple sclerosis presented. Multiple tools were employed in this challenging pathology. The second third left divisions involved neuralgia, while division was right facial side. Controlled radiofrequency thermocoagulation for isolated division, then radiosurgery conducted hemifacial After one month, because persistence pain attacks peripheral authorizations performed. Control pain, with withdrawal (BNI scale class I), achieved multi-therapeutic approach. Radiofrequency thermorizotomy performed very acute, immediate relief achieved. Pain regressed after radiosurgery, continued alcoholization control. should be treated dedicated neurosurgeon, avoiding techniques same using neurosurgical according interested intensity pain.
