Memory outcome after temporal lobe epilepsy surgery: corticoamygdalohippocampectomy versus selective amygdalohippocampectomy

ABSTRACT
The aim of this study was to compare IQ and memory outcomes at the 1-year follow-up in patients with medically refractory mesial temporal lobe epilepsy (MTLE) due hippocampal sclerosis. All were treated using a corticoamygdalohippocampectomy (CAH) or selective amygdalohippocampectomy (SelAH).The data 256 who underwent surgery for MTLE retrospectively evaluated. One hundred twenty-three CAH (63 [right side] 60 [left side]), 133 an SelAH (61 72 side]). A comprehensive neuropsychological test battery assessed before 1 year after surgery, results compared between surgical procedures. Furthermore, seizure outcome Engel classification scheme.At follow-up, there no statistically significant difference approaches respect outcome. Overall, scores showed improvement, but verbal decreased left SelAH. Verbal impairment seen left-sided resections especially cases SelAH, nonverbal right-sided resection, CAH. Left-sided produced some improvement memory. Older age longer duration seizures, greater frequency poor control associated poorer memory.Both can lead several cognitive impairments depending on side surgery. authors suggest that optimal type approach should be decided case-by-case basis.
