ABSTRACT
Many people with diabetes have permanently elevated blood sugar concentrations and a high level of diabetes-related psychological stress, also called "diabetes distress." In clinical practice, distress is often an impediment to successful self-management. psy-PAD psychodynamically oriented short-term therapy program whose goal reduce improve glycemic control.A randomized controlled trial was conducted 143 patients either type 1 or 2 who were being treated in eleven specialized diabetological practices. the intervention group (eight sessions) compared optimized standard care as control condition. The inclusion criteria HbA1c ≥ 7.5% combined (PAID >35, doctor's determination). primary endpoint at six months (t1). Diabetes-related (PAID), depressive symptoms (HADS-D, PHQ-9), anxiety (HADS-A), health-related quality life (SF-36), panic (short form PHQ-D), body mass index (BMI), triglyceride levels secondary endpoints. Follow-ups (t1) 12 (t2) (trial registration: DRKS00003247).The intergroup comparison t1 revealed significant, clinically relevant reduction by -0.53 percentage points (95% confidence interval [-0.89; -0.16], p = 0.005). analyses differences point estimators for t2, t2 BMI t1.For do not achieve satisfactory despite intensive treatment practices, integrated psychosomatic-psychotherapeutic can lower over intermediate term one-year period.
