ABSTRACT
Background: The management of Guillain-Barré Syndrome is very crucial for the outcome patient.
 Objective: aim study was to compare efficacy IvIg(Intravenous Immunoglobulin) versus PE(Plasmaexchange) in treatment mechanically ventilation adults with GBS neuro-intensive care unit Bangladesh.
 Methodology: Thiswas a prospective, observationalcohort study, Neuro-ICU from 2017 2018. We included all patients who required mechanical (MV). defined two groups: group 1 (group treated by IvIg: 0.4 g/kg/day 5 days) and 2 PE: PE during 10days, every alternate day). collectedclinical therapeutic aspects outcome.
 Results: A total number 49 (34 15 2) were enrolled. mean age 37.4±9.2 years, male predominance (65.3%). on electrophysiological findings, 4(32.7%) had acute inflammatory demyelinating polyradiculoneuropathy (AIDP) motor axonal neuropathy (AMAN) 26 (53.1%) motor-sensory (AMSAN) 3(6.1%)and NCS not done 4(8.2%) cases. length ICU stay 20±19.10 days 46.60±30.02 IVIG respectively. significantly shorter (p = 0.001) IvIg than group. Patients receiving early weaned MV 0.002) compared those statistical significance. Also, duration M/V (P=.002), Need tracheostomy (p=.005) over surval rate (p=.007) favoue Out patients, 3 died they AMAN variety.
 Conclusion: Our work reveals meaningful difference duration, stay, weaning excellent recovery terms less complcations.
