Transradial Retrograde Percutaneous Coronary Intervention of Chronic Total Occlusion via an Ipsilateral Septal Collateral Using a Single Guiding Catheter: A Case Report

ABSTRACT
With the development of specialized equipment and retrograde technique, success rates for percutaneous coronary intervention (PCI) chronic total occlusions (CTOs) have increased from 60 to 90% in past 10 years. Performing PCI via a collateral channel contralateral artery, using two guiding catheters, is usually preferred approach CTO-PCI. In case described this report, only ipsilateral septal artery proximal occluded left anterior descending (LAD) was available. The procedure can be performed successfully radial access single catheter.A 57-year-old patient, with history inferior myocardial infarction previous PCI, underwent planned arteriography due his complaints typical angina symptoms. Coronary angiography revealed stent occlusion located mid-LAD severe in-stent restenosis distal right (RCA). A branch supplying LAD retrogradely. After repeated failed attempts at antegrade LAD's CTO, tried. This finally succeeded through collateral. It radial-artery throughout whole process. Post-operatively, patient had no complications stable 1-year follow-up.The transradial CTO an catheter feasible safe appropriately selected cases.
