Serum Cystatin C Predicts Stroke Clinical Outcomes at 1 Year Independent of Renal Function

ABSTRACT
Objective: Serum cystatin C (CysC) is a sensitive marker of renal function to predict cardiovascular diseases. We aimed investigate the predictive value CysC for clinical outcomes independent in patients with acute ischemic stroke (AIS). Methods: measured serum levels 10,256 AIS from Third China National Stroke Registry (CNSR-III). The primary outcome was combination all-cause mortality and major disability (modified Rankin scale score, 3–6). Secondary included recurrence combined vascular events at 1 year. Outcomes were analyzed using logistic regression Cox proportional hazards models, respectively. Results: median 0.95 mg/l (interquartile range, 0.83–1.10 mg/l). A U-shaped association observed between (all-cause or disability) [quartile (Q)1 vs. Q2: adjusted odds ratio (aOR) 1.29, 95% CI 1.06–1.58, p = 0.012; Q3 aOR 1.12, 0.93–1.35, 0.242; Q4 1.35, 1.10–1.65, 0.004]. similar trend also existed “preserved function” patients. Adding model containing conventional risk factors improved performance integrated discrimination improvement (IDI) 0.13% ( 0.016) net reclassification index (NRI) 13.10% <0.001) outcome. No significant event rate different quartiles. Conclusions: showed correlation 1-year outcome, suggesting that may not only be simple candidate function.
