ABSTRACT
Introduction Advancements in genomic profiling led to the discovery of four major molecular subgroups medulloblastoma (MB), which have now been incorporated into World Health Organization classification central nervous system tumors. The current study aimed determine prognostic significance MB among children Malaysia. Methods We assembled samples from <18 years between January 2003 and June 2017 pediatric oncology centers was sub-grouped using 850k DNA methylation testing at German Cancer Research Centre, Heidelberg, Germany. Results Fifty patients diagnosed treated as were identified. Two (4%) 50 patients’ tumor insufficient for analysis. Of remaining 48 patients, 41 (85%) confirmed MB, while 7 (15%) results discrepant with histopathological diagnosis various other diagnoses. 15 stratified standard-risk (SR), 16 high-risk (HR), ten infants (age <3 old). Molecular subgrouping whole cohort revealed (14%) WNT, 11 (27%) SHH, 10 (24%) Group 3, (39%) 4. Treatment abandonment rates older 22.5% 10%, respectively. After censoring treatment abandonment, SR 5-year event-free survival (EFS) overall (OS) 43.1% ± 14.7% 46.9 15.6%, respectively, HR, EFS OS both 63.6% 14.5%. Infants had a 55.6% 16.6% 66.7% 15.7%, WNT tumors best 5y-OS, followed by 4, SHH ≥3 old. In younger children, showed favorable outcomes. Conclusion highlights importance diagnostic accuracy. Most their comparable those reported high-income countries. Due relatively small high rate, definite conclusions cannot be made regarding MB. Implementing this high-technology investigation would assist pathologists improving provide permitting subgroup-specific therapies.
