Prediction of survival after neoadjuvant therapy in locally advanced rectal cancer – a retrospective analysis

ABSTRACT
Purpose The aim of this retrospective analysis was to determine if the response preoperative radio(chemo)therapy is predictive for survival among patients with locally advanced rectal cancer and may act as a potential surrogate endpoint disease free overall survival. Results Eight hundred seventy-eight from five centers were analyzed. There 304 women 574 men; median age 64.7 years. 77.6% 22.4% received neoadjuvant radiochemotherapy or short-course radiotherapy, resulting in pathological complete 7.3%. T-downstaging N-downstaging occurred 50.5% 37% after therapy. In T-downstaging, 10-year DFS OS 64.8% 66.8% compared 37.1% 45.9% without T-downstaging. resulted 56.2% 62.5% 47.3% 52.3% N-downstaging. Based on routinely evaluated clinical parameters, an absolute risk prediction calculator generated 5-year disease-free survival, Conclusion radiotherapy better response. DFS, can be predicted using calculator.
