Yigu decoction regulates plasma miRNA in postmenopausal osteoporosis patients: a randomized controlled trial

ABSTRACT
Background Postmenopausal osteoporosis (PMOP) is a serious condition that affects elderly individuals. Our previous study revealed Yigu decoction (YGD) effectively improved bone mineral density (BMD) in individuals, but the mechanism underlying this effect remains unclear. In study, we investigated relationships among YGD, microRNAs (miRNAs), and metabolism by assessing effects of YGD on miRNA levels patient plasma to provide scientific basis for treating PMOP with YGD. Methods clinical trial, 60 patients were randomly assigned group or control (ratio 1:1) treated 3 months. The primary outcome measure was BMD, secondary measures included levels, visual analogue scale (VAS) scores, alkaline phosphatase (ALP) anti-tartrate acid (TRACP-5b) traditional Chinese medicine (TCM) syndrome scores. We assessed regulatory roles miRNAs analysing publicly available data from Gene Expression Omnibus (GEO) database. Bioinformatics methods also used explore which regulates are involved metabolism. Results Compared those before treatment, ALP TRACP-5b TCM scores VAS both groups after months treatment (P < 0.05). A total 82 differed between groups. After GEO database, confirmed miR-133a-3p key molecule mediates intervention PMOP. GO analysis genes suggested gene enrichment more pronounced response hormones, cellular growth factor stimulation, positive regulation physiological metabolic processes. KEGG these enriched mainly PI3K-Akt, FOXO, JAK-STAT pathways other pathways. results protein‒protein interaction (PPI) network epidermal receptor (EGFR), Insulin-like 1 (IGF-1), Caveolin-1 (Cav-1) others core proteins. Conclusion This demonstrated beneficial PMOP, ameliorating symptoms turnover indices. Moreover, inhibition expression may be mechanisms although multitarget multipathway manner.
