ABSTRACT
Introduction Usually caused by compression of the wrist's median nerve, carpal tunnel syndrome (CTS) is one most common types peripheral neuropathy. Tophi deposited under epineurium nerve compress leading to CTS, which very rare. Case presentation We report a 64-year-old man with history tophaceous gout who presented typical CTS symptoms and was admitted our hospital. A physical examination revealed swelling over right volar aspect region, he unable flex due subcutaneous rigidity. Tinel's sign Phalen's maneuver were positive. Electrophysiological studies confirmed diagnosis CTS. release surgery remove gouty tophus wrist performed when serum uric acid levels within normal limits (5.8 mg/dl). During operation, tophi found, completely removed. Operative findings gout. The patient recovered uneventfully without signs recurrence during 1-year follow-up period. Conclusion an uncommon cause may be if there evidence extrinsic or emanating from tunnel.
