ABSTRACT
Granulomatous lobular mastitis (GLM) is characterized by nonspecific chronic inflammation concentrated in breast lobules. Surgical resection one of the most common treatment options for GLM. On basis our previous use Breast Dermo-Glandular Flap (BDGF), we designed a new surgical approach GLM, especially cases where focus close to nipple. Here describe this approach.In Peking Union Medical College Hospital (PUMCH) and Beijing Dangdai during January 2020-June 2021, enrolled all 18 GLM patients who underwent surgery with Dermis-Retained BDGF. All were women; 18-50 years old (88%); clinical manifestation was mass (60%). Then, collected analyzed data about outcomes (drainage tubes moving time, relapse, patients' shape satisfaction). We regarded recurrence on same side as relapse. If there no complication patient's satisfaction excellent or good, rated successful. recorded occurrence postsurgical complications breast.The debridement area 3-5.5 (4.3 ± 0.7) cm; time 78-119 (95.6 11.6) min; mean (27.8 8.9 min) shorter than obtain transplant flap (47.5 12.9 min). Blood loss less 139 ml. As bacterial culture, two had positive results, but they symptoms. No surgery-related happened. In terms outcomes, drainage removed 5 days, only patient experienced relapse after 1 year follow-up. The follows: (50%), good (22%), acceptable poor (6%).For refractory conservative therapy former unsatisfactory management whose lesion vicinity nipple larger 3 cm, BDGF suitable fill after-debridement defect below nipple-areola achieve relatively satisfactory cosmetic outcome.
