Thiamine responsive Pulmonary Hypertension among exclusively breastfed babies –A Hospital based prospective study from southern India

ABSTRACT
Pulmonary hypertension remains leading cause of mortality despite many advances in its management. Thiaminedeficiency is identified as a reversible pulmonary exclusively breastfed infants whosemothers are thiamine defecient. The present study planned to find out the hypertensionin association with deficiency.Objectives: To pattern and associated factors due deficiency.Methodology: It hospital based tertiary care tumkur. Pretested pre designedquestionnaire was used collect patients socio demographic data, clinical data regardinginvestigations treatment prognosis.Results: Total 300 babies were included after taking into consideration all inclusionand exclusion criteria. All babies, (100) belonged Hindhu community. 234 (79.3%) class3 economic status according modified kuppuswamy classification. Majority 212 (70.6%) had onlyprimary education 32 (10.6%) illiterates. 88(29.3%) come severe gasping. 287(95.6%) babieshad hepatomegaly oliguria 144 (48%) babies. majority presentedwith shock 256 (85.3%), which managed. Repeat echo done serially difference reduction ofpulmonary pressure administration statistically significant (p value <0.005) Conclusion:Thiamine responsive presents an acute condition respiratory distress,vomiting previously well breast fed baby. Hence suspicion recognise symptomsearly diagnose mere life saving reverses fatal condition. Healtheducation mothers family members about food taboos related use polished rice prevent deficiencyof also important supplementation prophylaxis can be considered theprogrammes antenatal postnatal care.
