ABSTRACT
To evaluate the concordance between preoperative European Society for Medical Oncology (ESMO)-European of Gynaecological (ESGO)-European SocieTy Radiotherapy and (ESTRO) risk classification in early-stage endometrial cancer (EC) assessed by biopsy magnetic resonance imaging (MRI) with this based on histology surgical specimen.This bicentric retrospective study included women diagnosed EC (≤stage II) who had a complete assessment underwent management from January 2011 to December 2018. Patients were preoperatively classified into 3 degrees lymph node (LN) involvement MRI. Based final histological report, patients re-classified using classification. Concordance definitive was calculated weighted Cohen's kappa coefficient.A total 333 coefficient 0.49. The underestimated overestimated 37% 10% cases, respectively. Twenty-nine percent an incomplete LN staging according degree re-classification. observed discordance attributed MRI 75% 18% 7% both (p<0.001). Kappa 0.25 0.73 biopsy.Concordance ESMO-ESGO-ESTRO is weak. Given that majority wrongly classified, sentinel procedure instead no dissection could be option offered low-risk decrease indication second surgery re-staging and/or avoid toxicity adjuvant radiotherapy.
