ABSTRACT
Achondroplasia is the commonest form of short-limbed dwarfism and occurs in 1:26,000-40,000 live births. This an autosomal dominant disorder with abnormal endochondral ossification whereas periosteal intramembranous are normal. The basic abnormality a disturbance cartilage formation mainly at epiphyseal growth plates base skull. anesthetic management achondroplastic dwarfs challenge to anesthesiologist. Both regional as well general anesthesia have their individual risks consequences. We report case dwarf whom combined spinal epidural was used for fixation fractured femur. patient had undergone previous femur surgery under since he been informed that could be very problematic. There no technical difficulty encountered during procedure adequate level achieved low-dose local anesthetics without any problem. Postoperative pain relief offered three consecutive postoperative days using tramadol. discuss issues highlight role this patient. approach also helped early ambulation relief.
