Feasibility and efficacy of endoscopic reintervention after covered metal stent placement for EUS-guided hepaticogastrostomy: A multicenter experience

ABSTRACT
Although the use of a long metal stent is favored for EUS-guided hepaticogastrostomy (EUS-HGS) relief malignant biliary obstruction (MBO), endoscopic reintervention (E-RI) at time recurrent (RBO) challenging due to intragastric portion. This study evaluated feasibility and safety E-RI after partially covered (L-PCMS) placement during EUS-HGS.We performed multicenter retrospective between January 2015 December 2019 examining patients with MBO who underwent RBO through EUS-HGS route L-PCMS placement. Technical clinical success rates, details E-RI, adverse events (AEs), patency, survival were evaluated.Thirty-three eight referral centers in Japan enrolled. The location was distal 54.5%. median length 5 cm. As first attempt, via end existing successful 60.6%. overall technical rates 100% 81.8%, respectively. Liver abscess noted one patient. A proximal stricture associated ineffectiveness multivariable analysis (odds ratio, 12.5, P = 0.04). patency duration 140 394 days, respectively.Our findings suggest that technically feasible clinically effective, without any severe AEs, especially MBO.
