ABSTRACT
Dear Editor, Cerebral venous sinus thrombosis (CVST), a rare form of stroke, represents 0.5%–3% all stroke cases.[1] In various studies on CVST, the most common presentations are headaches, followed by seizures, focal neurological signs, altered sensorium, blurred vision, and vomiting.[2] Due to its variable presentation, CVST can be challenging diagnose without high level suspicion. We performed study that aimed analyze complete neuroophthalmological spectrum CVST. our experience, headache was prevalent clinical complaint in 93.33%, papilledema 40.0%, cranial nerve involvement 10.0%, diplopia 6.66% patients. The neuro-ophthalmological symptoms due intracranial hypertension occurring A rise pressure (ICP) causes axoplasmic flow stasis optic fibers prelaminar area head, which turn disc edema papilledema. When assessed using Friesen's grading, majority patients belonged Grades 4 5. Vision is normally unaffected early stages unless there an exudation, retinal hemorrhage, or macular edema. Newer diagnostic methods, such as optical coherence tomography, have made it easier identify subclinical. many 31% cases, untreated result secondary atrophy, leading permanent vision loss.[3] Thereby, prompt diagnosis intervention with presenting visual manifestations better prognosis. study, 43.3% presented blurring vision. (63.33%) had acuity less than equal 0.5 log MAR. frequent field defect found subjective tests concentric enlargement physiologic blind spot. detected 22.72% There two basic theories why physiological spot enlarges: first, acquired peripapillary hyperopia caused elevation retina subretinal fluid, second, compression lateral displacement fiber layer.[4] Sixth palsy, particularly susceptible elevated ICP because extensive path orbit, cause diplopia, observed To conclude, we therefore recommend identified cases confirmed magnetic resonance venography undergo comprehensive ophthalmological examination along serial monitoring, prevent damage.
