Older residents of long-term care facilities (LTCFs), also known as nursing homes, or residential aged facilities, often have multiple health conditions and are exposed to polypharmacy. Use high-risk medications such opioids, glucose-lowering medications, antithrombotics, antipsychotics is prevalent among LTCFs.
