ABSTRACT
Penetrating cardiac injuries represent a great challenge for the surgeon because of their lethality.It is estimated that more than 90% mortality happens before patient reaches hospital and survival after stab wounds to heart about 50% as opposed 11% gunshot wounds.A 58-year-old hemodynamically unstable male was brought ED with two left chest.Two surgical approaches were applied treat injury.First, urgent antero-lateral thoracotomy ventricle suture repair performed.Few hours later, due massive bleeding hemodynamic instability, through median sternotomy extracorporeal circulation arrest performed.Heart can be treated approaches.Left feasibly in emergency department but provides better access all important cardiovascular structures.Prompt early diagnosis associated well-trained team crucial management life-threatening injuries.
