Impact of a web‐based antimicrobial approval system on broad‐spectrum cephalosporin use at a teaching hospital

ABSTRACT
Objective: To achieve sustained improvement in use of cefotaxime and ceftriaxone (CEFX) a major teaching hospital, as measured against national antibiotic guidelines. Design setting: Pre- post-intervention survey CEFX the Royal Melbourne Hospital, tertiary hospital Melbourne, Victoria. Intervention: Web-based antimicrobial approval system linked to guidelines was developed by multidisciplinary team implemented March 2001. Main outcome measures: Change rate (defined daily doses [DDDs] per 1000 acute occupied bed days) over 8 months pre- 15 post-intervention; concordance indication for with post-intervention. Results: decreased from mean 38.3 DDDs/1000 days pre-intervention 15.9, 18.7 21.2 at 1, 4 Concordance rose 25% courses 51% within 5 (P < 0.002). Gentamicin also increased, 30.0 48.3 = 0.0001).
