Comparison of cannulated screw and dynamic hip screw for the treatment of femoral neck fractures

ABSTRACT
Objective: We aimed to compare the results of surgeries for intracapsular femoral neck fractures with cannulated screws(CS) and dynamic hip screw(DHS), due lack evidence which implant internal fixation is better . Methods: In our clinic between September 2005 November 2009, 38 patients were operated transcervical fracture collum femoris between17 65 years age. Eighteen DHS(47.4%) 20 CS fixation(52.6%). Results: 16 (42.1%) female 22 (57.9%) male mean age was 37.13 (17-65) years. The duration follow-up 18.05 (2-57) months. 26 in 1st -3rd day (68.4%), 9 4 7 (23.7%), 3 after 7th (7.9%). DHS group, (50%) had avascular necrosis (AVN), 6 (33.3%) failure, (16.7%) delayed union, 5 (27.8%) nonunion, 1 (5.6%) patient infection, myositis ossificans. According criteria Salvati Wilson joint assessment, group 8 (44.4%) very good, moderate. score evaluated as average 28 points (16-40). İn (40%) AVN, (5%) (15%) cases nonunion infection detected. Salvati-Wilson, group13 (65%) them are (25%) 2 (10%) assessed 33 (18-40). none failure (p<0,05). Conclusion: Except high rate detection method, no significant difference complications functional two groups.
