植込み型除細動器とリードの組み合わせにより，同一リードを用いてもペーシングのリードインピーダンスが異なる場合がある．また，ショックリードの状態把握について，微小電流を用いたパルス放電によるインピーダンスを用いる機種や，コイルと心室先端電極を含めたリードの導通テストを用いる機種があり，デバイス製造各社で統一されていないのが現状である．症例は，高インピーダンスリードを植込み型除細動器の初回植込み時より使用され，電池消耗のため2回の機種変更が行われた．使用された計3種類の植込み型除細動器間ではリードチェックの導通経路がそれぞれ異なり，測定値に違いを認めた．ペーシングリードのインピーダンス高値からリード断線が疑われたが，機種変更にて同一リードを継続して使用可能であることが判明した症例を報告する．
