Disorders of the Ras/mitogen-activated protein kinase (MAPK) pathway have an overlapping skeletal phenotype (e.g. scoliosis, osteopenia). The Ras proteins regulate cell proliferation and differentiation neurofibromatosis type 1 (NF1) individuals osteoclast hyperactivity increased bone resorption as measured by urine pyridinium crosslinks [pyridinoline (Pyd) deoxypyridinoline (Dpd)]. Pyd Dpd are hydroxylysine-derived collagen found in cartilage excreted urine. is most abundant bone. aim this...
Seventy-one cases of congenital constriction band syndrome (CCBS) were reviewed. The year birth, sex, ethnic background, gestational history, family and concurrent diagnoses obtained. anatomic patterns involvement, degree impairment, number type surgical interventions recorded. incidence CCBS appears to be rising. average patient had three involved limbs, with a predilection for distal, central digits the upper extremity. Abnormal histories found in 60% cases; 50% diagnoses, nearly one-third...
Background The RAS/MAPK disorders [Noonan syndrome, cardiofaciocutaneous (CFC) Costello and Leopard syndrome] are heterogenous conditions with phenotypic overlap. Their orthopaedic manifestations not well defined, their similarity makes differentiating them difficult. Methods We prospectively evaluated 60 individuals: 26 Noonan 32 CFC 2 syndrome. Each individual underwent a structured history physical evaluation by an surgeon, syndromic geneticist. Results All groups had high prevalence of...
Abstract Introduction : RASopathies are a group of genetic conditions due to alterations the Ras/MAPK pathway. Neurocutaneous findings hallmark features RASopathies, but musculoskeletal abnormalities also frequent. The objective was evaluate handgrip strength in RASopathies. Methods: Individuals with (e.g., Noonan syndrome, Costello cardio‐facio‐cutaneous [CFC] and neurofibromatosis type 1 [NF1]) healthy controls were evaluated. Two methods tested: GRIP‐D Takei Hand Grip Dynamometer Martin...
Abstract Thus far, the phenotype of tetrasomy 18p has been primarily delineated by published case series and reports. Findings reported in more than 25% these cases include neonatal feeding problems, growth retardation, microcephaly, strabismus, muscle tone abnormalities, scoliosis/kyphosis, variants on brain MRI. Developmental delays cognitive impairment are universally present. The purpose this study was to fully describe at both genotypic phenotypic levels. Array CGH performed 43 samples...
Jeune syndrome (JS) often results in lethal thoracic insufficiency syndrome. Since 1991, vertical expandable prosthetic titanium rib Dynamic PosteroLateral Expansion Thoracoplasty was used at our institution for treatment of JS. This study assesses the safety and efficacy this procedure.Twenty-four JS patients were treated, 2 lost to follow-up, 17 with a minimum 2-year follow-up retrospectively reviewed clinical course: Assisted Ventilation Rate, respiratory rate, capillary blood gases,...
In response to historic guidelines suggesting limits the amount of limb lengthening, we report results and complications those patients in whom initial goal lengthening exceeded 20% segment length. Thirty-one underwent a total 35 attempted lengthenings with mean follow-up 38.6 months. Limb-length discrepancy was primary indication all but one case. With an average 35% original bone length, achieved gain 33%. Lengthening within 2.5 cm 31 (89%) cases, significantly more successful goals...
Kyphoscoliosis is considered a relative contraindication to treatment with the Vertical Expandable Prosthetic Titanium Rib (VEPTR(®); Synthes Inc, Paoli, PA). Nevertheless, patients do present early-onset kyphoscoliosis and thoracic insufficiency syndrome, no suitable alternative treatments are currently available. However, it unclear whether VEPTR(®) reasonable for treating kyphoscoliosis. We determined controls progression in and, if so, what methods might be used improve control of...
A review of 106 cases Legg-Perthes disease disclosed 26 in whom a particularly poor radiographic appearance was seen. Hinge abduction detected 19 these patients and suspected but not proven the remainder. found to occur early course Legg-Perthes, hinging frequently occurred about an unossified portion femoral head, making detection difficult. Failure movement lateral corner epiphysis under edge acetabulum on internally rotated abducted radiograph is prima facie evidence hinge abduction....
Summary A review of 106 cases Legg-Perthes disease disclosed 26 in whom a particularly poor radiographic appearance was seen. Hinge abduction detected 19 these patients and suspected but not proven the remainder. found to occur early course Legg-Perthes, hinging frequently occurred about an anossified portion femoral head, making detection difficult. Failure movement lateral corner epiphysis under edge acetabulum on internally rotated abducted radiograph is prima facie evidence hinge...
Tibial fractures are relatively frequent injuries of young children. These occasionally the cause a childhood gait disturbance. Additionally, tibial may be result child abuse. Consequently, pediatrician should have knowledge concerning these injuries. In this paper, we present review in hospitalized children and describe their associations with nonaccidental trauma (NAT). Finally, provide suggestions for improving nomenclature clarity isolated spiral tibia.
Patients admitted to the authors' institution with tibial osteomyelitis between 1978 and 1998 were reviewed. The purpose of this study was determine incidence, etiology, treatment, outcome in chronic pediatric osteomyelitis. authors describe their treatment children resistant previous therapeutic modalities. Thirty patients identified a mean age 8.5 years at diagnosis. Mean admission 9.6 years, indicating more than 1-year duration disease. Follow-up averaged 2.3 years. underwent 97...
The radiographic changes in 57 patients with Legg-Perthes disease were studied to define the patterns of progression one stage next. Femoral neck width, epiphyseal width and height, acetabular depth, "tear-drop" distance, femoral head sphericity anteroposterior projection measured on 529 X-ray films. Two distinct groups hips found. Group I (43) showed moderate all parameters during fragmentation phase but improved subsequent years, maintaining a concentric reduction adequate head. II (14)...
