Summary Background: The American Society of Anesthesiologists (ASA) Grading System is widely used to describe preoperative physical status. Inconsistency grading between anesthetists has been demonstrated in studies using hypothetical adult patient scenarios. We aimed investigate the use and interrater reliability ASA pediatric anesthesia practice. Methods: A two‐part questionnaire was mailed all 176 current members Paediatric Anaesthesia New Zealand Australia (SPANZA). first part obtained...
Patients suffering from the mucopolysaccharidoses, most commonly Hurler's syndrome, present special problems in anaesthesia. A retrospective review of anaesthetic management such patients over fifteen years revealed a high incidence airway problems. Two case histories illustrating these difficulties are presented.
An experiment was designed to investigate the effects of arteriovenous (AV) fistula occlusion on cerebral autoregulation. A right carotid-jugular created in 63 rats such a way as produce an intracranial AV with loop extension into neck. The occluded after 8-week interval under both normotension and metaraminol-induced hypertension, evidence blood-brain barrier disruption investigated Evans blue dye technique. results indicate that may cause steal which is responsible for reduction threshold...
A prospective, randomized, double-blinded, placebo-controlled clinical trial was conducted in 41 patients evaluating the effect of a single preoperative dose intravenous dexamethasone on postoperative vomiting and pain children undergoing elective tonsillectomy. Dexamethasone found to significantly reduce incidence first 24 hours postoperatively (P = 0.02), time intake solids 0.001), need administer rescue antiemetic 0.005) fluid therapy requirements 0.006) period. No significant difference...
Children with cystic fibrosis (CF) receive general anesthesia (GA) for a variety of different procedures. Historical studies assessing risk GA report high morbidity. There is paucity data evaluating the currently available anesthetic agents. The aim this study was to assess effect on clinical status and lung function children CF.Children CF aged 8-18 years admitted IV antibiotic treatment pulmonary exacerbation were invited participate. Spirometry, forced oscillation technique (FOT), score...
Arthrogryposis multiplex congenita has been linked with malignant hyperthermia. A review of the anaesthetic experience at Royal Alexandra Hospital for Children over a 32-year period revealed no episode hyperthermia occurring in patients arthrogryposis despite many and varied exposures to known triggering agents.
We report out initial experience with intraoperative transoesophageal echocardiography (TOE) in 200 patients undergoing repair of congenital heart disease. Complications associated probe insertion precluded a full study 11 (5.5%) and included airway obstruction (n=6), inability to pass the (n=4) vascular compression (n=1). The preoperative diagnosis was confirmed by TOE 176 189 cases (93%) minor variances 12 (6.3%) one major variance (additional large muscular ventricular septal defect...
Previous studies of parental presence at induction anaesthesia in children have examined reaction to being present and the benefits gained from their presence. This study was undertaken specifically assess parents' attitudes child's anaesthetic. A total 154 questionnaires were distributed over a two-week period parents presenting Day Stay Unit for procedures requiring general anaesthesia. Eighty-eight percent would liked be induction. Only 41% expected only 40% actually Of those present, 94%...
We report three cases of propranolol overdose in two patients, both whom suffered cardiovascular collapse after receiving an emetic compound a hospital's casualty department. Such may be caused by vagal activity the presence sympathetic beta-blockade. recommend administration atropine, which increase tone, before patient receives compound, or undergoes gastric lavage endotracheal intubation.
Day stay paediatric tonsillectomy is well established in many parts of the world but not Australia. This audit presents protocol and results first one hundred twenty-five patients managed this way at our hospital. Patients assessed as being low risk postoperative complications were offered procedure a day patient. All had standardized relaxant anaesthetic technique with an intraoperative opioid antiemetics. The observed for six hours postoperatively Stay Unit contacted following surgery to...
Providing anaesthesia for the separation surgery of conjoined twins presents unique challenges to managing anaesthesiologists. The low incidence such surgeries and anatomical variations in each type makes a experience. This report features anaesthetic plan faced performing set thoraco-omphalopagus rural secondary hospital remote location India.
We report a fatal complication from central venous cannulation in child undergoing heterotropic liver transplantation. Following the attempted placement of cannula left internal jugular vein, extravasation blood products via resulted haemothorax, hypotension and eventual brain death. Possible causes strategies for prevention this are discussed.
We undertook a survey of N.S.W. hospitals and tertiary paediatric in other States to determine their practice relation parental presence at induction anaesthesia children. There were 135 responses 174 questionnaires. Twenty-one indicated that no children anaesthetized institution one was inadequately filled out. One hundred thirteen questionnaires assessed. Only 44% departments had an official policy on attendance. A quarter all described facilities as entirely suitable, half compromised...
Summary The Jervell and Lange‐Nielsen syndrome is an autosomal recessive trait characterized by deafness electrocardiographic changes. These changes include prolongation of the QT interval T‐wave inversion. Treatment may use β‐blockers to prevent development malignant ventricular arrhythmias sudden death. We report a patient with this syndrome, who was receiving propranolol orally became hypoglycaemic during preoperative fasting period prior cochlear implantation.
