Acute pancreatitis (AP) is a rarely encountered complication in patients following kidney transplantation and associated with significant morbidity mortality. The diagnosis often difficult due to the lack of classic symptoms laboratory findings. Herein, we report case azathioprine-induced AP leading acute injury transplant recipient, whom have diagnostic difficulty. Patient spontaneously recovered after drug withdrawal. We propose step-wise algorithm for evaluation recipients.
