大腸癌の発育過程を知る目的で, 手術摘出標本にmicroangiographyを行い, 大腸ポリープと癌の血管構築像を形態別に分類して検討した.大腸ポリープの血管構築は基本的に樹枝状, 扇状, 柵状, 混合型に分類され, 腫瘍の異型性に準じた改築がみられた.有茎性ポリープは樹枝状, 扇状を呈し, 柵状型はIIa, IIa+IIc, 混合型はIIa集籏型であった.癌化率は対全病変20%で, 腫瘍径に比例して高くなる傾向がみられた.腫瘤型pm癌はポリープの樹枝状, 扇状型に類似し, stalk invasionの像がみられた. ss (a1) 以深では血管系の改築に伴う微小循環樟害性病変がみられ, 潰瘍型の頻度が高い.潰瘍型進行癌は柵状型からの水平方向進展と樹枝状, 扇状型からの垂直方向進展が推察され, 後者は約65%にみられた.大腸ポリープの血管構築の特性とその改築様式は腫瘍の増殖に対応して大腸癌の発育, 進展に大きく関与することを強調した.

症例は66歳の女性で,突然の上腹部痛,嘔気を主訴として受診した.初診時腹部単純CTで胃拡張症と診断し,内科入院で保存的加療を行った.症状の改善乏しく,CT再検して脾臓が胃の背側へ遊走して,胃前庭部が左上腹部に捻転して押し上げられていた.上部消化管造影検査でも幽門部が高位に存在しており,遊走脾に伴う胃軸捻転症の診断となった.内視鏡的整復を試みたが困難であり,手術目的に当科紹介となった.胃短軸捻転症で胃体部後壁に壊死による穿孔を認め,胃軸捻転解除術,胃腹壁固定術,大網被覆術を施行した.術後経過は良好で胃潰瘍もほぼ改善し,第13病日で退院した.胃軸捻転症はまれな疾患であるが,胃拡張を呈した急性腹症の診断の際には念頭に置くべき疾患であると考えられた.
34例の同時性大腸多発癌について臨床病理学的検討を行い, さらにnow cytometerを用いて核DNA量を測定し以下の結果を得た.多発癌の占居部位では34例中15例 (44%) で同一領域内に発生しており, 特にS状結腸に多発していた.多発癌の第2癌は第1癌に比べ肉眼型で0型, 壁深達度で粘膜癌, 粘膜下層癌, 組織型で高分化腺癌が多かった.予後は単発癌の5年生存率が68%に対し, 多発癌では75%であった.核DNA量を測定した14症例37病変中, DNA diploidyは27例, aneuploidyは10例であったが, ploidypatternと肉眼型, 組織型, 壁深達度の間には有意差を認めなかった.各症例におけるDNA ploidypattemの組合せは, diploidyどうしが7例, diploidyとDNA aneuploidyが4例, DNAaneuploidyどうしが3例であり, aneuploidyどうしの組み合せでは, そのDI値は異なっており, 同時性大腸多発癌の各病変が独立した起源であることが示唆された.
Thirteen cases of breast phyllodes tumor have been experienced and 3 them had local recurrence.
