要旨:症候性頭蓋内動脈狭窄の再発率は10~18% / 年と高く,治療抵抗性の疾患である.その病態は多様であり,各病態に対する治療方針はそれぞれ異なる.近年の薬物治療の進歩によって頭蓋内動脈狭窄のイベント率は低下傾向にある.これに対し,血行再建術には現時点で優位性を示すエビデンスはない.頭蓋内動脈狭窄ではmisery perfusion を呈する症例は少なく,EC/IC bypass の適応は限られ,頭蓋内動脈用ステントもその有効性は示されていない.しかし,real world では積極的にこれらの介入が行われていると懸念される.外科治療を行う際には,病態が明確であり,「積極的」薬物治療にも抵抗性の症例のみを対象とすべきで,明確なエビデンスが示されていない現状では,best medical treatment の進歩を十分認識したうえで,血行再建術の適応にはより慎重であるべきと考える.
