We examined twenty-five patients who had a unilateral or bilateral slip of the capital femoral epiphysis and determined degree anteversion thirty-nine involved hips with computerized axial tomography. Thirteen (eighteen hips) were seen at time original diagnosis (Group I), twelve (twenty-one one to seven years after operative treatment II). The mean amount for all was +1.0 +/- 8.2 degrees. Group-I -0.7 7.4 degrees Group-II 2.5 8.7 in both Groups I II less than predicted individuals same age....
One hundred forty-three displaced (type III) supracondylar fractures of the humerus in children were treated over a 5.5-year period with 17 (11.9%) having signs vascular impairment at time presentation. Rapid reduction and Kirschner wire stabilization without arteriogram was performed each case. In three cases which satisfactory blood supply to hand not present after reduction, circulation restored exploration brachial artery revealed two intimal tears one arterial entrapment. The remaining...
Sixty-six femur fractures sustained by children ages 4-14 years and treated with external fixation were reviewed retrospectively to assess factors influencing the incidence of refracture. The total rate secondary fracture was 12% (eight patients) including five recurrent at original site three through pin sites. After removal fixator, patients refractured one patient fractured a tract. Two sites while fixator still in place. Multivariate linear-regression analysis showed no correlation...
Despite intensive study of acute septic arthritis in infancy and childhood, poor outcomes continue to occur. Failures are often due delayed diagnosis inadequate therapy, but can be factors beyond physicians' control such as associated osteomyelitis occurrence during infancy. Recent advances the understanding management include: (1) demonstration beneficial effects continuous passive motion rabbits; (2) identification inflammatory mediators cartilage destruction their respective sources; (3)...

Summary Epidural analgesia and anesthesia are standard regional techniques in orthopaedic surgery of the lower extremities. Benefits epidural anesthetic infusions include excellent analgesia, minimal respiratory depression, no somnolence, decreased need for blood transfusion. Adverse effects pruritus, nausea, urinary retention, but methods have evolved to counter each adverse effect. A continuous infusion opioid bupivacaine was used as principal postoperative analgesic 71 young patients...

Summary: We performed a retrospective review of 34 humerus shaft fractures (HSFs) in children younger than 3 years to determine the frequency child abuse young with this injury. Data were obtained from hospital records (including previous and subsequent emergency, clinic, inpatient notes), radiographs, county child-protective services. Cases reviewed independently by four physicians classified as probable abuse, not indeterminate. Only 18% abuse. The history findings other fracture itself...
BACKGROUND Stevens-Johnson syndrome (SJS) is a rare dermatologic disorder that characterized by nonspecific flu-like prodrome with fever, malaise, myalgia, cough, rhinitis, and sore eyes, followed characteristic rash mucocutaneous manifestations. An uncommon fracture of the proximal tibial epiphysis is described in two cases. A flexion-type Salter II tibia resulting from a partially closed physis can be reduced easily and appears to have no long-lasting effects. Radiographic review adolescent knees showed that physeal closure proceeds posterior anterior, thereby making this particular more likely during phase development.
This Clinical Report was revised. See https://doi.org/10.1542/peds.2024-070074. Fractures are common injuries caused by child abuse. Although the consequences of failing to diagnose an abusive injury in a can be grave, incorrectly diagnosing abuse whose fractures have another etiology distressing for family. The aim this report is review recent advances understanding fracture specificity, mechanism fractures, and other medical diseases that predispose infants children. clinical will aid...
One-hundred-fifteen patients with orthopaedic injuries resulting from a reported fall piece of furniture at home were studied to define the relationship suspected child abuse associated this mechanism injury. One hundred-thirteen sustained fractures or dislocations and two impaled pencil needle. Each patient was evaluated by primary care physician an emergency room surgeon. Background: Orthopaedic surgeons working in the Americas may be consulted care of patients bitten by venomous rattlesnakes (genus Crotalus ), particularly with regard to possibilities compartment syndrome and soft-tissue destruction. Despite considerable evidence regarding safety efficacy antivenin treatment rattlesnake bites adults, controversy persists roles surgery envenomations children. Our hypothesis is that aggressive use just as effective safe for children it adults. Methods: We...
The pregnancy status of female teenagers presenting for orthopaedic surgery is often unknown. Anesthetics may have teratogenic or abortive effects. authors report the results a screening program unrecognized in 532 surgery. Five patients with positive urine human chorionic gonadotropin test were identified, prevalence 1 result 106 tested (0.9%). Surgery was cancelled each case.
Entrapment of tendons in pediatric forearm fractures has been reported rarely and often is diagnosed months to years after the initial injury. Tendon entrapments are occur more frequently distally than proximally. Two cases proximal entrapment flexor digitorum profundus an ulnar shaft fracture here. Early diagnosis permitted surgical release early restoration full function. These emphasize difficulty diagnosing this rare condition a young child need for careful digital range motion...
