We present here the case of a 70-year-old female who developed systemic peripheral gangrene in both her upper extremities (all fingers) and right foot due to severe septic shock requiring vasopressor therapy. Interestingly, patient's left remained spared from gangrenous changes possibly chronic external iliac artery occlusion thus lower concentration vasopressors that extremity.
