BACKGROUND AND OBJECTIVES: Although many adults experience resource-intensive and costly health care in the last year of life, less is known about these experiences children with life-threatening complex chronic conditions (LT-CCCs). We assessed hospital resource use by type number LT-CCCs. METHODS: A retrospective analysis 1252 LT-CCCs, ages 1 to 18 years, who died 2012 within 40 US children’s hospitals Pediatric Health Information System database. LT-CCCs were identified International...
Although medical marijuana (MM) may have utility in the supportive care of children with serious illness, it remains controversial. We investigated interdisciplinary provider perspectives on legal MM use cancer.
Objective and Background: Few previous studies have explored how pediatric palliative care (PPC) influences hospital utilization. We evaluated this among PPC recipients in a single center. Methods: This is retrospective cohort study of 109 patients ≥2 years age who received consultation at large quaternary children's from April 2009 to September 2010. assessed frequencies admissions emergency department (ED) visits, use intensive interventions, costs. Generalized estimating equations were...
Importance Robust quality measures to benchmark end-of-life care for children with cancer do not currently exist; 28 candidate patient-centered were previously developed. Objective To prioritize among parents who lost a child cancer. Design, Setting, and Participants This survey study was conducted using an electronic, cross-sectional discrete choice experiment (DCE) maximum difference scaling from January June 2021 in the US. In each of 21 questions DCE, participants presented set 4 asked...
There are no existing quality measures (QMs) to optimize end-of-life care for children with cancer. Previously, we developed a set of 26 candidate QMs. Our primary objective in this study was achieve stakeholder consensus on priority measures. We conducted an iterative, cross-sectional electronic survey, using modified Delphi method build among clinician and family stakeholders. In each the two rounds surveys, stakeholders were asked rate QMs 9-point Likert scale, basis perceived importance....
PURPOSE Data on end-of-life care (EOLC) quality, assessed through evidence-based quality measures (QMs), are difficult to obtain. Natural language processing (NLP) enables efficient measurement and is not yet used for children with serious illness. We sought validate a pediatric-specific EOLC-QM keyword library evaluate attainment among childhood cancer decedents. METHODS In single-center cohort of who died between 2014 2022, we piloted rule-based NLP approach examine the content clinical...
This study investigated the relationships of health beliefs and HIV/AIDS knowledge with frequency condom use among women childbearing age in four major Indian cities. Surveys were completed by 210 attending six primary care centers. Among sexually active (N= 139), 68% noted rare or no condoms during intercourse. Perceived benefits (p= .05) normative efficacy requesting .01) related to a greater use. About 54% knew that breast milk could transmit HIV, but fewer than third aware an...
Abstract Background : We previously developed stakeholder‐informed quality measures to assess end‐of‐life care for children with cancer. sought implement a subset of these in the multi‐center pediatric palliative (PPC) database. Procedures utilized Shared Data and Research database evaluate proportion childhood cancer decedents from 2017–2021 who, last 30 days life, avoided chemotherapy, mechanical ventilation, intensive unit admissions, > 1 hospital admission; were enrolled hospice...
Abstract Background Children with cancer are increasingly using cannabis therapeutically. Aim The purpose of this study was to determine the perspectives and practices pediatric oncologists palliative care physicians regarding use for medical purposes among children cancer. Methods A self‐administered, voluntary, cross‐sectional, deidentified online survey sent all in Canada between June August 2020. Survey domains included education, knowledge, concerns about cannabis, views on its...
Abstract We conducted a retrospective cohort study of 125 pediatric oncology patients who died in 2010–2014 to explore how healthcare utilization, palliative care (PPC) receipt, and end‐of‐life (EOLC) differed between enrolled early phase clinical trials (EP) those not (NEP). Baseline characteristics utilization did significantly differ groups. EP received PPC consultation closer death than NEP (median days before = 58 [interquartile range 16–84] vs. 85 [32–173]; P 0.04). Our findings...
Efficacy of liposomal doxorubicin (LD) in treating desmoid fibromatosis (DF) children has not been well evaluated. This retrospective case series examines five with progressive DF, treated LD. We report progression-free intervals (PFIs) and radiographic as clinical responses for each medication received. LD was tolerated, an average 4.5% reduction tumor size median PFI 29 months. Treatment conferred the longest all medical therapies pursued. Thus, is important treatment option DF pediatrics.
Participation in research offers families a sense of control and meaning pediatric cancer care. Gatekeeping limits progress—collaboration is key. #PediatricOncology #PalliativeCare #Research #PallOnc #pedonc #hpm #hapc
Abstract Background Medical marijuana (MM) is legal in 34 US jurisdictions. Yet, little known about patient and parent perceptions of MM pediatric cancer care. We examined attitudes, beliefs, experiences regarding among parents children with adolescent young adult (AYA) patients, to help frame future research initiatives. Procedure In this qualitative study, we conducted semi‐structured, one‐on‐one interviews AYAs at a comprehensive center. Interviews were audio‐recorded, transcribed, coded...
Children, adolescents, and young adults with hematologic malignancies tend to receive high-intensity end-of-life care (HI-EOLC), but sociodemographic hospital-based predictors of HI-EOLC remain unclear. The authors conducted a population-based, retrospective cohort study the Premier Healthcare Database. They identified individuals who were 0 39 years old at death died between 2010 2017. was defined as experiencing 2 or more following: cardiopulmonary resuscitation, intravenous chemotherapy,...
We propose that renal salvage is possible in neonatal inferior vena cava (IVC) and bilateral vein thrombosis utilizing early surgical intervention. Management of with impairment typically includes anti-coagulation thrombolytic therapy. report a case five day-old premature female, found to have had propagated up the IVC, associated anuric failure. Peak BUN/creatinine was 52/4 at day life 5, peak 90/7.4 10. After failed attempt intravenous therapy, thrombectomy performed for IVC as well both...
