声帯溝症に対するvocal rehabilitationは, きわめて困難といわれてきたが, 対症的に声帯の容量を増加させ, 新たな声帯遊離縁を作る方法の一つに声帯内注入術がある.われわれは自家脂肪を注入物質として用い, 良好な結果を得たのでその詳細について報告するとともに, 声帯内脂肪注入術における今後の改良点と声帯の溝に対するアプローチに関する検討も加えた.2例の声帯溝症症例に対して, 全身麻酔下に, 経皮的自家脂肪注入術を施行し, 術後の音声の評価を行った.術後音声は, 自覚的, 他覚的に改善した.しかし, 注入術は, 注入後の脂肪吸収による効果の持続に問題があり, 移植術も考慮すべきであることや, 溝に対する直接的なアプローチとして, 溝を切徐術し, トラニラストを投与することも, 音声のなお一層の改善に寄与できる実験結果を示した.

発声時声門閉鎖不全による音声障害の非観血的手術に声帯内注入術がある。近年注入物質として自家脂肪を用いる報告がみられるようになったが,その方法や効果に対する詳細な報告がない。そこで,われわれは,脂肪の声帯内注入術を追試し,その結果について報告する。症例は,一側喉頭麻痺症例8例と声帯溝症2例で,全例,経口挿管による全身麻酔下に,腹部皮下より直接あるいは市販の脂肪採取器を用いて脂肪を採取した。注入は14Gアンギオカットを用いて経皮的に施行した。注入後の音声は,注入1ヵ月後に評価し,全例に最長発声持続時間の延長がみられた。

発声時仮声帯も振動する仮声帯発声について, 自験例の音声治療経過を過去の文献と比較し報告した.症例は66歳の男性で, 嗄声を主訴とし来院した.軽度の声帯溝症を伴った仮声帯発声の診断で, 喉頭ファイバースコープを用いた視覚的フィードバック法および音声訓練を実施した.初診より90日後の最終評価では仮声帯の接近は解消され, 声帯による声門閉鎖が得られ, 音声も良好となった.GRBAS評価においては全般的嗄声度Gは2から0へと改善し, ソナグラムの分析でも倍音波形の振幅が大きくなり, 櫛型の明瞭な調波構造となった.本例の仮声帯発声の原因は声帯溝症による声門閉鎖不全を代償するものと考えられた.ファイバースコープでの発声運動の視覚的フィードバック, 音声治療手技が有効であった.当院の仮声帯発声の症例は発声障害患者400例に対して5例, 1.2%であり他の文献と一致していた.

音声外科手術後に生じる肉芽形成や瘢痕形成は,術後音声を大きく左右する。そこで,肥厚性瘢痕治療薬であるトラニラストの局所投与による肉芽組織形成抑制効果を実験的に検討した。方法:0.5%カラゲニン誘発刺激による肉芽組織およびイヌ声帯新鮮創に対して,2.0%トラニラストおよびリポソーム封入2%トラニラストを局所投与し検討した。結果:2%トラニラストに比較して,リポソーム封入2%トラニラストは組織内に長く止まり,肉芽組織の増生抑制効果をより促進した。また,イヌ声帯新鮮創に対して局所投与した結果,術後に良好な声帯振動が得られた。結論:トラニラストの局所投与により,肉芽組織の増殖抑制効果が認められ,さらにリポソームに封入した形態にして投与することにより組織内濃度が維持され,肉芽組織の増生抑制効果がより増強された。
