先天性心疾患に合併する肺出血の多くは体肺動脈側副血管（aorto-pulmonary collateral artery, APCA）によるものだが，ファロー四徴症の術後遠隔期に特発性肺ヘモジデローシスと診断された症例を経験したので報告する。症例は4歳，女児。2歳時にファロー四徴症修復術を受け，周術期に原因不明の貧血を繰り返した。肺出血により呼吸不全を来した際にはAPCAに対する経皮的コイル塞栓術を施行したが効果は乏しく，支持療法で徐々に改善した。4歳時に両側びまん性肺出血よる貧血と低酸素血症を来し集中治療を受けた。胃液と気管支肺胞洗浄液の細胞診でヘモジデリン貪食像を認め，他の肺出血の原因となる疾患は否定的であるため特発性肺ヘモジデローシスと診断された。ステロイドと免疫抑制剤の投与により回復し退院した。先天性心疾患に合併するびまん性肺胞出血において，出血源となるAPCAがないにもかかわらず肺出血を繰り返す場合には，特発性肺ヘモジデローシスを念頭において診療にあたることが望ましい。
