Sixty-three children out of a total 199 patients seen with cutaneous tuberculosis during 7-year period were included in this study. Culture was positive only four, and the diagnosis based on clinical examination, tuberculin reaction, histopathology, response to antitubercular therapy. Forty had lupus vulgaris (LV) 23 scrofuloderma (SD). The lower half body predominantly affected those LV, keratotic hypertrophic forms frequently encountered. LV planus mainly face. Ulcerative atrophic types...
Adherence to antiretroviral therapy is difficult, and methods increase it are needed. We tested the impact of supportive telephone calls in an adherence substudy a treatment trial. Subjects initiating received either each site's usual support measures or scripted serial (16 during 96 weeks).A total 282 subjects enrolled: 140 group 142 group. A 75% expected were completed. Virologic failure occurred 97 (34%) subjects: 52 (37%) those 45 (32%) group; time virologic was not different (P=.32). In...

Abstract Tetralogy of Fallot with absent pulmonary valve syndrome is a unique congenital condition often accompanied dilated arteries causing varied degree airway compression. Frequently, abnormal arterial branching at the segmental level and compression intrapulmonary bronchi can be seen. The occurrence isolated bronchopulmonary malinosculation in form malformation exceedingly rare. This report highlights case such rarity, involving young child recurrent lower respiratory tract infections.
Kartagener syndrome (KS) is a rare autosomal recessive condition with triad of situs inversus, bronchiectasis, and sinusitis. In majority cases, the heart structurally normal; however, rarely intracardiac anomalies can be seen in patients KS such atrial or ventricular septal defects pulmonary stenosis. We present coexistence mesocardia, corrected transposition great arteries young adolescent male diagnosed on computed tomography angiography.
Objectives: Complex antiretroviral regimens can be associated with increased toxicity and poor adherence. Our aim was to compare the efficacy safety of switching two simplified, class-sparing regimens. Methods: We conducted a randomized, open-label study in 236 patients virologic suppression who were taking three- or four-drug protease inhibitor non-nucleoside reverse transcriptase regimen for ≥ 18 months. Patients received lopinavir/ritonavir (LPV/r) 533 mg/133 mg twice daily + efavirenz...

Objective: Whether therapeutic drug monitoring of protease inhibitors improves outcomes in HIV-infected patients is controversial. We evaluated this strategy a randomized, open-label clinical trial, using normalized inhibitory quotient (NIQ), which incorporates exposure and viral resistance. NIQs ≤ 1 may predict poor outcome identify who could benefit from dose escalation. Design/methods: Eligible had load ≥1000 copies/ml on failing regimen, began new inhibitor containing regimen at entry....
Objective: We evaluated the safety, tolerability and antiretroviral activity of β-D-2,6-diaminopurine dioxolane (DAPD; amdoxovir) with or without mycophenolate mofetil (MMF) in HIV-1 infection following extensive therapy (ART). Methods: Oral DAPD 500 mg twice daily placebo MMF was added to failing ART. RNA viral load (VL) decline week 2 analyzed by intent-to-treat, using rank-based tests. Patients VL > 0.5 log10 copies/ml at (virologic response, VR) optimized ART continued for up 96 weeks....
