Iatrogenic dissection of the common carotid artery (CCA) is a feared complication transcarotid revascularization (TCAR). However, its management not well described. We describe our treatment approach to CCA during TCAR in four patients, and discuss risk factors, prophylactic measures, an algorithmic management.
Background Botulinum toxin-A (BTX) has numerous cosmetic and therapeutic applications. Our previous studies have found that BTX augments pedicled flap survival through both vasodilatory effects attenuation of the inflammatory response to ischemia in rat. This study examines effect chronic on microcirculatory vascular tone its acute topical vasodilators muscle flaps. Methods The spinotrapezius Sprague-Dawley rats underwent a single 2-week pretreatment 0.2 mL saline either with (n = 5) or...
The aging population, in combination with the popularity of breast augmentation implants, presents surgeons a growing number cases involving women undergoing minimally invasive cardiac surgery (MICS) who have implants. We present an unusual complication delayed migration subpectoral implant into chest cavity through iatrogenic defect after mitral valve repair. This wall was ultimately repaired latissimus dorsi flap. Although MICS has been described documented experience remains limited. Most...
Aneurysmal degeneration of the thoracoabdominal aorta after aortic dissection is a well-documented sequela Marfan syndrome (MFS). Hybrid technique (HT), an emerging treatment modality for complex pathologies, decreases morbidity and mortality relative to open surgery. However, outcome data regarding HT in genetic aortopathies such as MFS limited. We describe case young male with hypertension type B (AD) complicated by symptomatic aneurysm (TAAA). He underwent staged comprised carotid-carotid...
A 16-year-old girl with Loeys-Dietz syndrome presented an acute, complicated type B aortic dissection (AD) mesenteric and right renal malperfusion owing to a dynamic obstruction. The anatomy of her AD genetic aortography were suboptimal for thoracic endovascular repair. Given the concern anticipated late degeneration need open repair, she underwent successful transfemoral septal fenestration stenting into superior artery additional artery. Her failure angina resolved, was discharged home....
A 74-year-old man with pancreatic cancer had undergone pancreaticoduodenectomy and subsequently developed ischemic hepatitis secondary to high-grade celiac artery stenosis. Celiac antegrade stenting via brachial access was unsuccessful, open bypass would have required takedown of the and/or biliary anastomoses for adequate exposure. Retrograde was, therefore, successfully performed gastroduodenal stump. His resolved, he ultimately discharged dual antiplatelet therapy. Computed tomography...
A 55-year-old man with autoimmune deficiency syndrome presented an infected left ventricular pseudoaneurysm and sepsis. The aneurysmectomy consisted of a Dor-style pericardial patch plus debridement the abscess cavities. recurred, much larger, within five months due to persistent second repair was done without omental flap. No recurrence has occurred one year after repair, but patient asymptomatic, partial gastric herniation inside pericardium. This is first description primary...
