Activated partial thromboplastin time (aPTT) is the primary test used to monitor intravenous (IV) direct thrombin inhibitors (DTIs) but has many limitations. The plasma diluted (dTT) shown better correlation with DTI levels than aPTT. This study compared dose-response curves for dTT and aPTT in pediatric patients receiving argatroban bivalirudin.A retrospective review of treated (n = 45) or bivalirudin 14) monitored aPTT.The assay was calibrated report concentrations µg/mL good analytic...

Abstract Background Rho(D) immune globulin (RhIg) is used to reduce RhD alloimmunization in pregnancy. This study describes potential causes for after the development and implementation of RhIg. Study Design Methods retrospective descriptive investigated RhD‐negative patients born 1965–2005 with anti‐D newly identified during 2018–2022. Transfusion, pregnancy, intravenous drug abuse, transplantation were considered sources. Results There 1200 (852 females; 348 males) at 30 institutions 5...

Abstract Alloimmunization is prevalent in patients with sickle cell disease (SCD) and can be a barrier to gene therapy (GT) due the necessary transfusion support for successful stem collection infusion. We estimate that standard‐of‐care GT an adult SCD will require average of 35–45 units red blood cells over 6‐month period. Institutions should actively plan these needs share information inform national consensus policies on management alloimmunization during GT.

Abstract Objectives This study aimed to enhance medical education by integrating virtual reality (VR) tours into the clinical pathology curriculum, comparing VR with traditional didactic methods. Methods Seven 360° were developed for Microbiology and Transfusion Services laboratories. A controlled crossover involved 171 students (83% MS4) from April 2021 2023. Students randomly assigned either or PowerPoint (PP) presentation groups. Surveys assessments measured understanding, interactivity,...

Abstract Background The United States military has transfused room temperature platelets (RTPs) in combat since 2004, but distribution to far‐forward locations is limited. To expand access, cold‐stored (CSPs) were approved for 10‐day storage 2017, increased 14 days 2019, clinical outcome reporting been This study assessed whether CSPs associated with mortality trauma patients when compared RTPs. Study design and methods A retrospective analysis of databases evaluated casualty records...

Abstract Background Trauma‐induced coagulopathy (TIC) is associated with negative outcomes. Pediatric TIC has been described most often in older children. Children undergo normal developmental hemostasis, but it unknown how this process impacts the risk of across childhood. Aims To understand variations coagulation testing and pediatric age groups. Methods We evaluated patterns studies at presentation over first 72 h hospitalization by group a large, Level I trauma center, 2015–2020. The...

Abstract Due to the global SARS‐CoV‐2 pandemic, in‐person laboratory medicine clerkships were converted distance learning. The remote clerkship format provided advantages of allowing participation students from more locations and greater scheduling flexibility but new challenges maintaining learner engagement providing experiential content environment. Increased physical activity is protective against worsening of postthrombotic syndrome (PTS) in adults. We assessed patient eligibility, consent, adherence, and retention rates a pilot trial prescribed following venous thromboembolism (VTE) children. Secondary objectives were to describe the within-subject changes PTS, quality life, coagulation biomarkers before after intervention each group. enrolled randomized patients between 7 21 years age group or standard care (education-only) 1:1...

Abstract Background Delayed hemolytic transfusion reactions (DHTRs) are reported to be rare occurrences but may more frequently observed in the trauma setting where patients heavily transfused, followed over long inpatient admissions, and have frequent subsequent blood counts as they undergo multiple surgical interventions. Study design methods We examined rates of DHTRs on a per patient basis an academic county hospital with level 1 center serving four‐state region 3‐year period. were...

Abstract Objectives We asked whether age or injury severity drives blood use patterns in paediatric trauma. Background Transfusion for trauma care is complicated by known developmental differences coagulation and patterns. Methods/Materials linked 10 years of Trauma Registry bank data, 2011–2020, all acute patients aged <18 treated at a large US Level 1 adult centre. assessed age, mechanism association with any use, within the first 4 h care, resuscitation balance, using grouped‐age...

Low-titer group O whole blood (LTOWB) is attractive for acute trauma care as it delivers concentrated and balanced hemostatic resuscitation in single large bags. Whether cold-stored LTOWB can sustain platelet counts unclear.Four cohorts of patients-three historic, one retrospective-were identified by their urgency use. Admission all subsequent over the first 24 h were compared with t-tests. The follows: 1292 patients at Maryland Shock Trauma described Stansbury colleagues 2013; 35 enrolled...

Abstract Background Administering platelets through a rapid infuser is proven to be safe. However, the clinical significance of infusing ABO‐incompatible with red blood cells (RBCs) in remains unclear. There theoretical risk that isoagglutinin plasma platelet unit can interact RBCs and induce hemolysis. Abstract Background Expired blood can be transfused if clinically indicated but outcome data do not exist. We hypothesized that modestly outdated effectively support a hemorrhaging patient until surgical control is achieved. Abstract Background Use of prophylactic platelet transfusions in the neonatal setting is a common practice due to concerns about bleeding despite evidence developmental mismatch with transfusion adult platelets. The Platelets for Neonatal Transfusion-Study 2 (PlaNeT-2) published January 2019 showed that using higher thresholds (50 x 109/L) was associated more adverse outcomes and no improvement events compared lower (25 109/L).
