In Brief BACKGROUND: Emergence from general anesthesia can be associated with coughing, agitation, and hemodynamic disturbances. Remifentanil may attenuate these responses. METHODS: a prospective, double-blind, randomized trial, we enrolled 60 adult patients undergoing nasal surgery using remifentanil-based anesthesia. During the emergence phase, remifentanil group had reduced to one tenth of maintenance rate, whereas control discontinued. RESULTS: Times awakening tracheal extubation were...

Multivisceral transplantation (MVT) is an extraordinarily complex surgical procedure that imposes significant physiological stress driven by hemodynamic instability, fluid shifts, and severe bleeding. These factors can profoundly affect cardiovascular function increase the risk of complications. Stress cardiomyopathy (Takotsubo syndrome), a transient non-ischemic heart failure characterized acute left ventricular dysfunction, recognized complication major surgeries but has not previously...

Steep Trendelenburg during surgery has been associated with many position-related injuries. The American Society of Anesthesiology practice advisory recommends documentation, frequent position checks, avoiding shoulder braces, and limiting abduction upper extremities to avoid brachial plexopathy. We conducted a web-based survey assess anesthesiologists' practices, institutional policies, complications encountered when using steep Trendelenburg. Two thousand fifty randomly selected active...

Background and Objective. End-stage liver disease is characterized by a precarious imbalance of hemostasis. Detrimental consequences hypofibrinolysis, also known as fibrinolytic shutdown, have been recently demonstrated, its significance in visceral (ie, an allograft that contains the intestine) transplant remains unknown. Design Setting. To fill this gap, following institutional review board approval, retrospective study included 49 adult recipients allografts (14 “visceral without liver”...

Complexity of combined heart-liver transplantation has resulted in low adoption rates. We report a case series adult patients receiving en-bloc (HLTx), describe technical aspects, and discuss benefits the technique.Retrospective review HLTx over 18 months, with clinical follow up to 1 year. Primary outcomes included postoperative mortality major complications. Secondary 1-year survival, cardiac or hepatic allograft rejection, infection.Five received HLTx. Mean recipient age was 43 years...

A growing number of surgical patients present to the operating room with implantable cardioverter defibrillators (ICD). Peri-operative care these dictates that ICD function be suspended for many procedures avoid inappropriate, and possibly harmful, therapy triggered by electromagnetic interference (EMI). An alternative reprogramming is use a magnet temporarily suspend its function. However, this approach not without complications. We report case where failed inhibit sensing EMI, shock was...

Advanced renal cell carcinoma (RCC) resection has important anesthetic management implications, particularly when tumor extends, suprahepatic, into the right atrium. Use of transesophageal echocardiogram (TEE) is essential in identifying extension and guiding resection. Latest surgical approach avoids venovenous cardiopulmonary bypass yet requires special precautions interventions on anesthesiologist's part. We present a case Level IV RCC resected without salvaged by TEE guidance detection...

Background. New-onset systolic heart failure (HF) after liver transplantation (LT) is a significant cause of morbidity and mortality; however, its characteristics are still insufficiently delineated. HF may involve the left ventricle (LV), right (RV), or both ventricles. We explored incidence, characteristics, etiologies, risks, involved cardiac chambers, outcomes LT. Methods. This study included 528 adult patients with preoperative LV ejection fraction ≥ 55% who underwent LT between 2016...

Congenital rubella syndrome (CRS) is caused by virus infection of the pregnant mother leading to teratogenic effects on fetus. Although rare in developed world, manifestations reach far into adulthood and underscore importance careful evaluation before surgery. We present a case an adult with CRS whom unexpected prolonged postoperative respiratory depression occurred. Perioperative workup investigations pertaining patient's insufficiency are discussed.
