Recanalization is considered a prerequisite for favorable outcome in basilar artery occlusion (BAO). Intravenous thrombolysis (IVT) has been successfully used eligible patients with BAO well beyond the 4.5-hour time window but largely underrepresented best medical management arms recent randomized controlled trials of recanalization therapy BAO. We aimed to analyze outcomes treated IVT only and compare endovascular thrombectomy (EVT).

Plasma glial fibrillary acidic protein (GFAP) and tau are promising markers for differentiating acute cerebral ischemia (ACI) hemorrhagic stroke (HS), but their prehospital dynamics usefulness unknown. We performed ultra-sensitivite single-molecule array (Simoa®) measurements of plasma GFAP total in a code patient cohort with cardinal symptoms [National Institutes Health Stroke Scale (NIHSS) ≥3]. Sequential sampling included 2 ultra-early samples, follow-up sample on the next morning. We 272...

The data on long-term outcome after basilar artery occlusion (BAO) are scarce. Little is known about BAO survivors´ over decades. We set out to investigate survival and causes of death in patients with up two decades follow-up. We also evaluated differences trends. Two hundred seven treated intravenous thrombolysis (IVT) at the Department Neurology, Helsinki University Hospital, between 1995 2016, were analyzed. Short-term was assessed by modified Rankin Scale (mRS) 3 months. Long-term...

Background: Around 30-60% of patients with basilar artery occlusion (BAO) present coma, which is often considered as a hallmark poor prognosis. Aim: To examine factors that will help predict outcomes in BAO comatose on admission. Methods: A total 312 angiography-proven were analyzed. Comas assessed Glasgow Coma Scale (GCS) ≤8 or impaired level consciousness ascertained the medical records. Outcomes evaluated modified Rankin (mRS) over phone call at 3 months. In our study, 53 excluded due to...

Objectives Blood-based biomarkers could enable early and cost-effective diagnostics for acute stroke patients in the prehospital setting to support initiation of treatments. To facilitate development ultra-acute biomarkers, we set out implement large-scale blood sampling determine feasibility diagnostic timesavings this approach. Background and purpose Patients suffering from basilar artery occlusion (BAO) treated with intravenous thrombolysis are, in some centers, started on adjunct anticoagulation hyperacute settings. We aimed to assess the outcome of such patients compare low‐molecular weight heparin (LMWH) unfractionated (UFH) this context. Methods examined 211 angiography‐proven BAO either UFH or LMWH. Main variables were rate symptomatic intracranial hemorrhage (sICH) according European Cooperative Acute Stroke...

Background Accurate identification of acute stroke by Emergency Medical Dispatchers (EMD) is essential for timely and purposeful deployment Services (EMS), a prerequisite operating mobile units. However, precision EMD recognition currently modest. Aims We sought to identify targets improving dispatcher identification. Methods Dispatch codes EMS patient records were cross-linked investigate factors associated with an incorrect dispatch code in prospective observational cohort 625 patients...

Basilar artery occlusion (BAO) leads to high rates of morbidity and mortality, despite successful recanalization. The discordance between flow restoration long-term functional status clouds clinical decision-making regarding further aggressive care. We sought develop validate a practical, prognostic tool for the prediction 3-month favorable outcome after acute reperfusion therapy BAO.This retrospective, multicenter, observational study was conducted at four high-volume stroke centers in USA...

Introduction After severe ischemic stroke (IS), circulating levels of symmetric dimethylarginine (SDMA) increase. We investigated the early dynamics SDMA in to potentially aid with prehospital identification IS from hemorrhagic (HS). Methods performed targeted mass spectrometry (MS) measurements two sequential acute plasma samples (early and secondary) 50 patients LVO 49 HS patients. Secondary 227 84 moderate symptoms (NIHSS ≥ 7) subsequently underwent ELISA validation. Results The median...
