【目的】維持血液透析患者において超音波エコーを用いた大腿前面筋厚測定の検者内信頼性, および男女別に大腿前面筋厚 (quadriceps femoris muscle thickness, 以下 : QT) と運動機能との関係を検討した. 【対象と方法】維持血液透析患者182例を対象に, 超音波エコーを用いたQT, 握力, 最大歩行速度を測定し, 筋厚測定の検者内信頼性は級内相関係数, 運動機能との関連を相関係数にて検討した. 【結果】筋厚測定の検者内信頼性は高く, 男性においてQTは握力, および最大歩行速度と有意な相関を認め, 女性においてQTは最大歩行速度と有意な相関を認めた. 【結語】超音波エコーによる維持血液透析患者の大腿前面筋厚測定は, 腎臓リハビリテーション領域における有用な指標になり得ることが示唆された.

炭酸ランタン(L)とスクロオキシ水酸化鉄(S)のリン吸着効果と,制酸薬内服によるその変動について明らかにするため,本法人4施設の維持血液透析患者163名を対象として調査した.2017年4月1日から2019年12月31日の間に,前述2種類の薬剤いずれかを内服開始・増量した症例を対象とし,背景因子と薬剤変更前後の血清リン濃度の変化量を調査した.S開始・増量群では副作用による内服中止症例が多かったが,250 mgあたりのリン低下量はLに比して有意に大きかった.両薬剤の250 mgあたりのリン変化量を制酸薬の有無で比較すると,Lでは制酸薬内服症例の変化量が非内服症例の約66%と有意に小さかったが,Sでは有意差を認めなかった.Lのリン低下効力は制酸薬による胃内pH上昇で減弱するが,Sでは減弱しないことが示唆された.制酸薬内服患者におけるリン吸着薬の選択や結果の解釈には十分な注意が必要である.
